Schedule a Senior Assessment
Perhaps the most important step in helping you live the life you love is making sure we know exactly what your medical condition actually is.
That’s why we want to look you over and determine your current health status. We call this your Senior Assessment, and it often involves several different healthcare professionals who will focus on the particular challenges you might be facing.
Why Get a Senior Assessment?
When we're traveling somewhere, it's always a good idea to have a map so there are no mistakes along the way. The Senior Assessment is like your personal "map of health" showing you, and us, the path to your maximum wellness and independence.
The Senior Assessment is important for every senior, even if you are currently being treated by a primary care physician whom you plan to continue seeing. We will be glad to share our in-depth assessment with your doctor, so he or she will have the benefit of our insights and thoughts on your health and wellness.
What Will the Assessment Reveal?
The assessment will cover many areas vital to you and your lifestyle. For instance, we want to find out what medications you're taking, and if you might be taking too many. The fact is, taking more pills doesn't necessarily make you healthier. In some cases, reducing your overall medications can actually improve your attitude and energy.
Other conditions included in your assessment, depending on your specific needs include:
- Analysis of your walking ability, to reduce your chances of falling
- Assistance with incontinence issues that restrict your independence and ability to enjoy life
- Monitoring of the various kinds of arthritis that could be limiting your movement
- Checking into mood swings and memory difficulties that may be a source of worry for you and your family.
What Happens After the Assessment?
Once we’re finished with your Senior Assessment and we review it with you, you’ll have a clear and detailed description of your overall health, along with suggestions on how you can start feeling better (or keep feeling as great as you already do!).
